New Patient Registration

couple_counsellingIf you live in our practice area and are entitled to use the NHS, you can register with us.

Come into the Health Centre with your completed registration form and two forms of identification these can include...

(one item of photo ID is to be seen, along with one document confirming your address).

  • birth certificate
  • marriage certificate
  • medical card
  • driving licence
  • passport
  • local authority rent card
  • paid utility bills
  • bank/building society cards/statements
  • National Insurance number card
  • payslip
  • letter from Benefits Agency/benefit book/signing on card
  • papers from the home office
  • P45.

 

You will be registered with the Practice rather than a specific doctor, but you still have the right to ask to see a particular doctor, if you wish. If you have a repeat prescription then you should arrange an appointment with the doctor as soon as possible.

We do not exclude patients on grounds of race, sexuality, religion, social class, age, disability or medical condition.
