Detailed Coded Record Access Form
As a parent you can request access to your child's online services. There are 2 different types of access as above. Parents can have Standard access to their child's online services until their 16th birthday however if a parent has access to their child's Detailed Coded Record, this access will end on the child's 11th birthday.
Access can be granted by completing and returning one of the forms to reception.
Parental Standard Access Form
Parental Detailed Coded Record Access
Proxy access is used when a patient consents to allowing someone else (a named person) access to their record. This access can be revoked at any time by the patient or if the GP has any concerns the access is being misused. Proxy access can also be granted by the GP if it is deemed as being in the patient’s best interests.
A child aged between 11 and 16 may use this form to grant parents access to their detailed coded record. This will be on the understanding that the child is competent to make the decision (decided by GP) and understands they can withdraw this at any time.
